Medical Clearance is typically NOT required unless otherwise specified by Dr. Raskin/Physician Assistant, Ashley Madsen. If medical clearance is required, it must be obtained within 30 days of your surgery date.

NO DENTAL WORK two weeks prior to surgery (this includes routine cleanings). We will advise you when dental work can be resumed following surgery.

BLOOD THINNERS: Alert Dr. Raskin/Ashley, if you take any blood thinners (i.e. Aspirin, Coumadin/Warfarin, Pradaxa, Plavix/Clopidogrel, Xarelto, etc.). **DO NOT STOP these medications unless you are directed to do so by your prescribing physician, surgeon or physician assistant. If these medications are stopped, please resume taking them the day after your surgery. You DO NOT need to stop anti-inflammatories (i.e. Ibuprofen/Advil/Motrin, Aleve/Naproxen, Mobic/Meloxicam, or Celebrex).

NO EATING or DRINKING after midnight the night before surgery (this includes water, cough drops, candy, gum, etc). No smoking cigarettes after midnight. No recreational drug use 7 days prior to surgery. On the morning of surgery, you may take routine prescription medications that you would normally take in the morning with a sip of water only. Please note you will be taking these on an empty stomach, therefore do not take any medications that would normally require food.

Contact lenses are NOT allowed to be worn during surgery. Please wear your glasses on the morning of surgery. The surgical center will not supply any patients with contact lens cases.

Wear loose comfortable clothing on the day of surgery with wide arm sleeves to fit over the post-op bandage. Do NOT bring jewelry, large amounts of money, or other valuables with you to the surgical center. We are not responsible for lost/stolen items.

You will be called by our office the day before surgery with the time of the procedures as well as the time to arrive at the surgical facility. If you do not hear from us by 4:30 pm, please call the office at 212-889-8600.

Arrange for someone to pick you up after your surgery; a post-operative escort is required for all patients.

ELEVATION Please keep your hand elevated above the level of your heart as much as possible for 1 week after your surgery; this includes bedtime. The foam Carter Block pillow will be given to you on the day of your surgery and will assist in protecting your upper extremity, as well as aiding in elevation. It should be used for at least 3 nights after your surgery, and then as needed to alleviate any swelling/pain.

The Carter Block pillow is the preferred method of elevation when sitting or sleeping. We will also provide you with a sling on the day of surgery. The sling should be used when in transit/outdoors and then removed. Daily shoulder range of motion (shrugs, circles, etc) is important to prevent any residual stiffness in the shoulder from sling use.

Begin finger range of motion exercises the day of surgery unless otherwise instructed. Any parts of your fingers that are contained within the bandage should not be exercised. All portions that are exposed should be worked on, including curling to make a fist and opening the fingers to straighten them up. Perform the exercises slowly in sets of ten approximately three times per hour. You may use the other hand to help the digits move if they are stiff or uncomfortable to actively move on their own.

Elevation and digit exercises are vital to reduce the swelling following surgery. It is normal to have mild to moderate swelling after surgery, however consistent elevation and regular performance of digit exercises will diminish the swelling. If the swelling is worsening rather than improving the first several days after surgery, contact the office.

The prescribed pain medication should be taken after arriving at home, or if you do not live within the city, before you leave the city. It may take several doses to notice improvement. Please take the medication only as directed. If the prescription pain medication is causing vomiting, stomach irritation, or other side effects, please discontinue use. Constipation is not uncommon when taking prescription opioid medications; in addition to proper hydration and fiber content, you may need to take an OTC agent (e.g. Colace, Miralax, Dulcolax).

Numbness and tingling of the fingers in the surgical hand is not uncommon. It is temporary and will resolve with time. Please limit your activity level to promote optimal healing. Keep the bandage clean and dry. Plastic bags may be used to cover the arm in the shower. Cast covers may also be purchased at most local pharmacies.

Oral antibiotics are not necessary after your surgery, unless otherwise instructed by Dr. Raskin or Ashley. Numbness/tingling in the fingers of your surgical hand is not uncommon, and expected if you had a nerve block to anesthetize your extremity. It is temporary and will resolve with time.

You will receive a call from Ashley within 24-48 hours of your surgery to discuss the post operative plan, answer any questions, and schedule your follow up appointment. In case of an emergency at any time, call our office at 212-889-8600.
